(If you have a handwritten waiver we will apply that the day of your consult. )
- Find and Click "Locations"
- Click "Book online"
- Check "New Patient" below "Book an Appointment"
- Select "In-person" or "Virtual Visit
- Select the date and time of the appointment
- Type "New Patient Consult" as "Reason for Visit"
- Fill in appointment information
- Review the privacy policies.
- Click "Continue"
- Describe your current issue or inquiry under "Additional Notes".
- Click Continue
- Once the appointment is approved and confirmed, you will receive a confirmation with your appointment information.
Once your appointment is confirmed, you will receive a second email with electronic intake forms.
- Complete these forms at your earliest convenience, prior to your scheduled consult/appointment.
- You will need a copy of our driver's license and insurance card.
