Will my health insurance cover my visit?

Our staff is experienced with health insurance, and although we are a self-pay practice, we will help you maximize your benefits. Some of the procedures and treatments we offer may be covered by your health insurance at your out-of-network benefits rate. We will be happy to provide you with the proper documentation and billing necessary for you to self-submit your office visits to your carrier for reimbursement. We work with many specialty lab companies, and will utilize insurance for lab tests. Since all insurance plans are different, we encourage you to proactively contact your carrier to verify your own out-of-network benefits.

Why does SLW choose to not participate in any insurance plans?

  1. Insurance company plans are generally designed to cover “sick care,” as opposed to the type of wellness care that functional medicine offers. Unfortunately, at this time, many insurance plans are not designed to recognize our type of patient care. 
  2. We believe in spending quality time with all patients to assure that a very high level of patient care is delivered. By being a self-pay practice, we get to see our patients for as much time as we need.
  3. Choosing to be an out-of-network provider allows us to perform multiple procedures or treatments during one office visit. SLW providers are at liberty to decide which treatment or procedures are best for each patient during each office visit.
  4. Functional medicine is on the cutting edge of many different types of treatments and testing. Some of our newer procedures and treatments may not be recognized as “standard” care according to certain insurance plans. We are hopeful that insurance plans will soon recognize functional medicine as a type of standard care. 

What forms of payment are accepted?
Cash, Debit, or credit card are all accepted methods of payment for services. We also accept HSA/FSA. When you schedule the initial visit, we require a credit card on file to hold the appointment. Charges will be applied if you miss or cancel an appointment without proper notice.

Where can I find pricing information?
Visit the Services drop down for the provider you are wanting to see. All pricing will be available within each page. 

Do you offer any specials or bundle discounts?
Currently our special offers can be found on our socials, Facebook, Instagram & LinkedIn. In the near future we do plan to offer comprehensive bundle pricing.  

Cancellation Policy
We require 24 hour notice. All last minute or same day cancellations will be billed at 50% of the visit fee. No shows will be charged the full appointment fee. 

I received a notification that my labs are in, how will I review my results?
Our office will call to schedule follow-ups when ALL lab results are returned. This can sometimes take 3-5 weeks. Always reach out via your patient portal if you have questions.
